for a new multi-specialty physician medical complex
and Personal Care Home expansion.
New three-story facility - expansion of the Personal Care Home and office space for Community Health Associates LLC.
On the hospital campus - corner of William Street and Pratt Avenue on the south end of the existing Personal Care Home facility.
FIRST FLOOR PLANS: Personal Care Home 20-Bed expansion to include 14 private rooms and 3 semi-private rooms.
SECOND FLOOR PLANS: Medical Office Building with 22 exam rooms to accommodate CHA general practice physicians.
THIRD FLOOR PLANS: Medical Office Building with 14 exam rooms and 4 procedure rooms to accommodate CHA specialists.
PARKING: 106 parking spaces.
TIMELINE: Projected completion date scheduled for late summer 2013.
