Memorial Hospital Home Health Agency provides quality health care services in the convenience and comfort of your home. Our experienced team understands your unique health needs and administers care under the direction of your physician. We are committed to providing you with a variety of services within your home, from prenatal, newborn and pediatric care to chronic, post surgery and disability. Whatever your needs, our Home Health team will design a plan that is specific to you and your situation.

An integral part of the hospital, our Home Health Agency has the knowledge and resources of our complete facility at its disposal. Whenever your health needs can be met at home, our staff is available, 24 hours a day, 7 days a week.

Nurses, physical therapists, speech therapists, occupational therapists, medical social workers, home health aides and nutritional counselors are available to provide treatment, monitor and assess conditions, and provide instruction to meet patients' needs. An individualized plan of treatment is established with a physician's approval.

  • NURSING SERVICES - Our qualified, competent nursing staff has the experience needed to make recovery at home a success. A registered nurse will assess the home-care requirements of the patient, then work with the patient's physician to develop a customized treatment plan. Our nursing staff is trained to provide quality care for the ill and disabled of every age while offering instruction and emotional support to the rest of the family.

  • PHYSICAL THERAPY - Services include assessment, therapeutic excercises, gait training, range of motion activities, and ultrasound.

  • OCCUPATIONAL THERAPY - Services include assessment, restoration of function in activities of daily living, task oriented therapeutic activities, and use of assistive devices.

  • SPEECH LANGUAGE PATHOLOGY - Speech services for all ages including treatment for swallowing difficulties, voice disorders, receptive and expressive language modalities, nonverbal communication, augmentative communicators.

  • MEDICAL SOCIAL SERVICES - Personal, family or financial concerns can prevent patients from achieving maximum physical recovery. Therefore, Memorial Hospital’s total patient care program includes the services of licensed Medical Social Workers who can provide you and your family with many services. Social workers are available to help you with any problems which might be troubling you when you return to your home. Some of the services available are:
    • Financial Information
    • Help with personal and family problems.
    • Help with adjustments to an illness.
    • Referrals to appropriate community services.
    • Information and arrangements for nursing home, intermediate and residential care.

    Do you feel confident living in your own home?
    Lifeline is a personal emergency response service that helps individuals remain safely at home. Individuals can have access to help 24 hours a day, 7 days a week. Simply pressing a personal alarm help button connects to a professional who will carefully and quickly assess the situation and link the caller to appropriate help.
    A service you can trust.
    One of our trained installers will come to your home to meet you, install the communicator, and make sure that you know how to use the service. The communicator and your personal help button are your links to Lifeline.
    How much does it cost?
    You pay a one-time installation fee, plus a monthly fee. There are absolutely no other charges or fees to pay. You can discontinue the service at any time, for any reason, and return the equipment.
