How do I make an appointment at Memorial Hospital?

Most services will be scheduled through your doctors office. Meet with your doctor to schedule an appointment for Radiologic, Sleep Studies or other services. As always, you do not need an appointment for an Emergency Room visit. If you are looking to establish yourself with a local healthcare provider, Community Health Associates may be the perfect local fit with offices in Towand, Dushore and Wyalusing. You can call Community Health Associates at 570-268-4713 to schedule your appointment.

Please arrive at least 30 minutes before your scheduled first appointment so that we can properly register you.

If you cannot keep your appointment, call 570-268-4713 to reschedule as soon as possible. Please provide at least 24 hours notice so that we may offer this appointment time to another patient.

What should I bring to my first appointment?

Please bring the following to your first appointment:

  • Bring your current medications with you or a list of your medications and dosages.

  • Bring your insurance card.

  • Your co-pay.

  • If you will be visiting Community Health Associates, bring copies of any medical records from your previous physician/hospital or ask your physician to fax them to 570-268-4716 prior to your visit. If you are uncomfortable asking your physician to send information to us, contact us at 570-268-4713 and we will contact your provider on your behalf.

How soon can I get in for my new patient visit?

Our providers will attempt to schedule your appointm,ent as quickly as possible. Normally, you can get an appointment within a few days. However, some providers may be booked out further but you can see one of the other providers in the clinic for your new patient visit.

How do I know which physician to select as my primary care physician?

I think I should see a specialist. How do I get a referral?

Patients may visit the Emergency Room for an emergent evaluation of your situation. If you would like to visit witha provider from Community Health Associates you must make an appointment with a provider in the practice to be examined before any referrals are issued. At times, the diagnosis can be resolved in the Emergency Room or with a primary care provider.

I'm from out of town and I'm running low on my prescription. What should I do?

You should plan to call your physician 3 days (or 72 hours) before you run out of any prescription. If your prescription has run out and you are still feeling ill you may need to be seen again. If it is an emergency visit the Emergency Room at Memorial Hospital. If you will be in town for a while, you should contact Community Health Associates by calling 570-268-4713 and ask to be connected to a nurse that can help you with a medication problem. Be sure to have the following information:

  • Your name, date of birth and daytime telephone number

  • The name, address, and phone number of your primary care physician that prescribed the original medication.

  • The name AND telephone number of your pharmacy

  • The name of your meds and dosages

Also, state how many days you have left in your prescription and be sure to state if you have run out of your medication.

I had a blood test and x-ray. When will I have the results?

Please give at least 48 hours for the tests to be processed, transcribed and sent to your providers medical group. Your physician will ask you to call sooner if he or she needs to discuss the tests with you.

I received a bill. Where can I call to discuss this?

If your bill is from Memorial Hospital please go here for additional information on your bill. If your bill is from Community Health Associates call 570-268-4713 with any questions.

Are you a walk in clinic?

You are always welcome to walk into the Emergency Room at Memorial Hospital. Community Health Associates is not a walk in clinic, if you are having immediate health issues, you should call 911. You will need to schedule a new patient visit with one of our providers to become an established Community Health Associates patient.
