SERVICES
Our Social Workers
work closely with residents and their families throughout their stay.
The social workers are available to provide individual, group, or family
counseling and act as a liaison and advocate for the resident. The social
workers also assist in facilitation a safe, planned discharge.

The social worker
assesses the needs of the patient and / or community need and can provide
the following services:
- Community referral
to appropriate agencies
- Education
- Crises intervention
- Counseling
- Spiritual support
referral

The residents of Skilled
Nursing and their families receive counseling to aid their adjustment
to placement, to enhance their quality of life, and for discharge planning.
