care can find the specialized therapies they need and the quality care they deserve.
The programs of this facility combine the advantages of specialized rehabilitation and
long term care with the services of our acute care hospital.
An individualized plan for around the-clock care is developed by our interdisciplinary care team including:

COMFORTABLE REHAB ACCOMMODATIONS
Rehabilitation is often a critical component of a resident’s care plan.
Under the direction of the resident’s physician, a skilled team of physical,
occupational and speech therapists evaluate the rehab needs of each resident
and designs an outcome-oriented treatment program.

Enhancing Quality of Life
The goal of specialized rehabilitation is to help residents attain their maximum
potential and restore the abilities and confidence essential to independent living.
Physical Therapists assist in body strengthening, mobility, flexibility, and gait
training to prevent injury and to help patients in their recovery.
Occupational Therapists assist in performing activities of daily living.
Speech Language Pathologists help with cognition, speech, and language to help patients and families communicate.
All therapists strive to maximize the patients potential and help families understand and assist with limitations.
DINING
Our residents enjoy socialization in one of our two dining rooms that feature buffet
style dining. The registered dietician visits with each resident to obtain food preferences and
develops their individualized menu. Staff is available to serve and assist residents at meals.
NURSING
In working closely with physicians, residents and their families, nursing is able
to offer the highest quality of care to enhance the quality of life of the residents.
SOCIAL SERVICES
Our Social Workers closely assist residents and their families throughout their stay.
Social workers are available to provide individual, group, or family assistance and
act as a liaison and advocate for the resident. The social workers also assist in
facilitating a safe, planned discharge.
RECREATIONAL AND SOCIAL ACTIVITIES
Residents are encouraged to participate in a wide range of activities such as parties,
movies, exercise classes, musical programs, religious services, craft classes, Wii
and entertainment experiences. The activity department also schedules outside excursions
to allow residents to stay in touch with the community.
ADMISSIONS
Admissions to the Skilled Nursing Unit are carefully considered for individuals who
require professional nursing care and related medical services on a 24-hour,
7 day a week basis while not currently in need of acute hospital care.
CONVENIENT LOCATION
The Skilled Nursing Unit is nestled in the Endless Mountains of Northeastern Pennsylvania
and is conveniently located just off Route 6 in North Towanda on the Memorial
Hospital campus in Bradford County.
